Vendor Booth Rental Documents

Vendors can sign up for booths online on each individual event page (see links below). It is simple just fill out the online forms, pick your site, and pay for the site at the same time.

If you just need one of the miscellaneous forms you can either fill them out online and they are emailed to Lake Breeze with a copy back to you OR you can download a copy and hand fill it out and either US mail it in or scan it and email it into to events@lakebreezeterrace.com.

It is important to note that we do not hold booths. All booths need to be paid for at the time of reservation.

Miscellaneous Forms

For your convenience, you have 3 methods to complete the forms, online (you will receive an email copy after online completion), download the form to your desktop or phone (print and US mail it to us or scan and email back to us), OR send me an email and I'll email it to you directly (you can print and US mail or scan it back to us.)

Forms to complete: (1) Vendor Booth Rental Agreement & (2) Booth Rental -- Waiver of Liability.
